NO SHOW APPOINTMENTS:  

A $50 fee will be assessed if appointments are not cancelled at least 24 before the scheduled appointment date and time. 

FIRST TIME PATIENTS:

If you are a first time patient and no-show a scheduled appointment without 24 hours notice, you will no longer be accepted as a patient. 

OUTSTANDING  BALANCES:

If you do not have a plan with the billing department and have an outstanding balance that is not paid within 180 days, you will be dismissed from the practice.

WAITING ROOM:

Given that we have a small waiting room, please respect the privacy of others and refrain from taking phone calls in the room. Kindly step out of the waiting room if you need to take a phone call.
